Membership scheme

Members are entitled to unlimited access to the following services at no extra charge:

• GP consultations and telephone consultations. The GP reserves the right to ask for a face to face consultation if they feel it is necessary;

• Repeat prescription (not including cost of medication from pharmacy);

• Completion of forms and letters;

• Annual health check (not including pathology or other tests required)

Please note:

Home visits, Laboratory investigations, ‘Out of Hours’ Consultations, vaccinations or any other treatments/tests are not included in the Membership fees and may incur an additional charge. Treatment not covered by the monthly or annual Membership payment should be paid for at the time of the consultation.

Please note: we can register in the membership scheme under 18 patients only if they have a parent/carer/legal guardian register as member too.

Minimum membership period is one year.

TERMS AND CONDITIONS FOR MEMBERS

1. Definitions

1.1 Welcome to Dr Fiona McAndrew, Private GP. By accessing our services, you agree to be bound by the following Terms and Conditions. Please read them carefully. These terms govern the relationship between you (the patient) and Dr Fiona McAndrew (referred to as “the Clinic,” “we,” “us,” or “our”).

Clinic: Dr Fiona McAndrew Practice its staff, affiliates, and facilities.

Patient: The individual receiving medical services or their legal guardian/representative.

Services: All medical, diagnostic, therapeutic, and administrative services provided by the Clinic.

Members: means the individual(s) accepted into the scheme

Membership fee: means the monthly or annual subscription fee payable for membership benefits.

1.2 It is a condition of Membership that Members agree to pay the fees referred to hereafter and agree to be bound by these Terms & Conditions. These Terms & Conditions may be revoked, altered or added to from time to time by the Clinic, without prior notification to Members. The current version of the Terms and Conditions will be displayed on the Clinic’s website.

 

2. Eligibility and Application

2.1 Membership shall commence on receipt of completed and signed forms. Membership will always run for an Initial Period of 12 months. After The Initial Period, cancellation will require 30 days written notice to the Clinic. Further details of the cancellation procedure are set out below.

2.2 Members are entitled to cancel this membership agreement within 15 days from the date of signing (the “Cooling-Off Period”) without providing any reason and without incurring any cancellation fees. To exercise this right, members must notify us in writing via email to info@fmcandrew.co.uk. If members have made any payments (including any joining fee, initial membership fee, or pro-rated amounts), we will refund them in full within 7 days of receiving their valid cancellation notice, provided they have not used the membership facilities beyond what is reasonably necessary to assess the service (e.g.: GPs’ appointments). After the 15-day period expires, standard cancellation terms as set out elsewhere in this agreement will apply.

2.3 While a Standard Health check is included in the Medical Membership packages. If additional or further tests may be requested, they will be discussed with the member first. These additional tests may incur extra charges. The health check can only be carried out with your assigned doctor.

2.4 Although Members may use the GP telephone consultation service, they should understand that the GPs may require a face-to-face consultation in line with their clinical judgement in order to make satisfactory and safe diagnoses and provide clinically appropriate advice. In the interests of continuously improving our service to our members, telephone calls may be recorded and/or monitored. Our practice now uses Heidi, a secure AI-assisted tool, to help with consultation recording and note-taking. This allows the doctor to focus fully on you during the appointment while ensuring accurate, detailed records for better ongoing care. All recordings are securely deleted after the notes have been checked and saved to your record. You can opt out if you prefer not to have your consultation assisted by Heidi—just let the doctor know at the start of the consultation (or contact the practice in advance). This won’t affect the quality of your care.

2.5 Membership is non-transferable and non-refundable, unless there is a written agreement to the contrary.

2.6 Child Members can only be accepted if a parent or legal guardian is also a Member. A child is defined as any person aged under 18 years. Child Member status will change to adult status when a child reaches 18 at which stage they will be required to become a Full Member and complete a Membership Application Form in their own right.

2.7 Members must give written notice to the Clinic of any change of address, telephone number and email addresses.

2.8 At all times, the Clinic reserves the right to refuse an application for Membership from any applicant in its sole discretion.

2.9 The Clinic will try to keep you informed of developments and changes in the Clinic’s membership schemes, products and services. If Members do not wish to receive information from the Clinic, the Member should inform the practice of their communication preferences.

3. Membership Fees and Payment

3.1 All monthly Membership plans must be paid by Worldpay. Payments will be taken monthly on the day that the membership was signed.

3.2 Annual membership will be paid in advance by debit/credit card or BACS.

3.3 Monthly Child Membership fees will be paid via their parents’ or guardians at the same time. Additional treatment costs are to be paid for at the time of treatment.

3.4 Treatment not covered by a particular Membership will be paid for at the time of the treatment.

3.5 For the avoidance of doubt, no pathology costs are covered by any Membership schemes.

3.6 A full price list of all aspects of treatment not covered by Membership plans is available from the Clinic upon request at any time.

3.7 If a membership is cancelled and restarted more than once, the practice reserves the right to require future renewals to be paid annually (in one full payment taken upfront).

3.8 Prescription-writing and repeat prescriptions are free of charge to Members. For the avoidance of doubt, however, the cost of medications prescribed is not covered by any of the Membership schemes.

3.9 Members will be advised of any increase in Membership fees if this occurs.

3.10 Members may be charged 50% of the usual consultation fee in respect of any GP appointment for which they do not attend the Clinic without at least 24 hours’ notice of cancellation or rearrangement to the Clinic.

4. Cancellation of Membership

4.1 Cancellation by the Member:

i. Members will be liable to pay a full year of Annual membership fees during the entire duration of their initial period of membership (one year). After the initial period (one year), Members shall be entitled to terminate their Membership with no less than one months’ notice.

ii. Request for cancellation must be made in writing and addressed to the Practice Manager via email: info@fmcandrew.co.uk.

iii. No refunds of that month or year’s Membership fee shall be made, unless there are extenuating circumstances in the view of the Company, and all other sums due to the Practice must be paid prior to termination of the Membership. In the event of an extenuating situation the action required will be at the discretion of the Practice.

iv. The notice period of one month will commence from the first day of the month after the month in which notification of cancellation is received. The Practice Manager will confirm receipt of the notification and confirm the cancellation date within 10 days of receipt of a written email. If Members do not receive confirmation of cancellation within 10 days, they should inform the Practice Manager immediately as it will always remain the Member’s responsibility to ensure that cancellation emails have been received.

4.2 Termination by the Clinic

We may terminate your membership immediately and without any refund if:

a. You fail to pay any sum due;

b. You act abusively, aggressively or inappropriately towards staff or other patients;

c. The clinician-patient relationship has irretrievably broken down;

d. You provide false information or misuse membership benefits.

5.     Liability

5.1 All General Practitioners practising at the Clinic will have appropriate professional medical indemnity for their work.

5.2 The Clinic will provide services with reasonable care and skill. However, we do not guarantee specific outcomes or results from medical treatments.

5.3 The Clinic is not liable for any loss, injury, or damage resulting from patient negligence, failure to disclose relevant information, or non-compliance with medical advice.

5.4 The company will not accept liability for any damage or loss to members’ or their guests’ personal property brought onto Practice premises.

5.5 A person who is not a party to the Contract will have no right under the Contracts (Rights of Third Parties) Act 1999 to enforce any term of these Terms and Conditions. These Terms & Conditions will be governed by English law and the parties submit to the exclusive jurisdiction of the English courts.

6. Force Majeure

6.1 Where the performance by the Clinic or its obligations under these terms and conditions is delayed, hindered or prevented by an event or events beyond its reasonable control and against which an experienced service provider could not be reasonably have been expected to take precautions, the Clinic shall promptly notify the patient, in writing, specifying the nature of the force majeure event and stating the anticipated delay in the performance of its service.

6.2 The Clinic shall have no liability to a Member or Non-Member for any failure to deliver the services or any delay in doing so for any damage or defect to the services provided that is caused by an event, including without limitation strikes and other industrial disputes, extreme weather, epidemic, pandemic, terrorist incidents, breakdown of systems or network access, flood, fire, explosion, earthquake, windstorm, other nature disasters, war, civil war, riots or Government action preventing the delivery of services.

7. Amendments

The Clinic reserves the right to update or modify these Terms and Conditions at any time. The most current version will be available on our website or at our facility. Continued use of our services constitutes acceptance of the updated terms.

8. Confidentiality and Data Protection

8.1 We process your personal and health data in accordance with UK GDPR 2018 and our Privacy Policy (available on our website)

8.2 It is necessary for the purpose of providing patients with health advice and support for the Company to retain data concerning members’ identities, contact details and general health on file. By signing the Membership Application Form or Patient Registration Form, you expressly Consent to the Company having authority to keep this data for the duration of your Membership and for up to two years afterwards; or if Membership is refused for any reason, for the period of two years from the date you completed this form to assist the Company to provide you with the best possible service.

If you have a complaint, please contact the Practice Manager Rejeane Baeza at rejeane@fmcandrew.co.uk. We acknowledge within 3 working days and we will investigate and respond within 20 working days.
